How to make a referral to Leeds AdvocacyIf you, or someone you know, might need some help from an advocate then you can either ring the Leeds Advocacy office (0113 2440606) to ask for a referral form or you can download a form from the referral form link above. The main crietia we have is that you must be an adult with a learning disability, with a connection to the Leeds Metropolitan area. The referral form asks for some basic information about the person who wants an advocate and what the problem is that they need help with. Once you have filled the form in and sent it back to the office you will get a postcard so that you know we have received the referral. Every Wednesday there is a meeting and the new referrals are discussed. An advocate will then be asked to go out to see that person and the referrer to find out a bit more information. The advocate will then come back to the next meeting to talk about what more information has been found out and suggest what service can be offered. If we can offer a service then that person will be matched up with an advocate for the work to begin. If, for some reason, we cannot offer a service then we will signpost you to an organisation that might be more suited to your needs. |
